How to not use Arduino

First off, I would like to apologize if this has been explained before, but I cannot come up with any search combinations leading me to my answer.  So, here goes...

I have completed a simple project using an Arduino Uno and breadboard.  I then drew this project in Fritzing.  I would like to get a PCB made of this project, but would like to keep the form factor small.  My question is, how do I take the Arduino out of the equation?  Would I have to replicate all the components on the Uno board into my custom PCB in order to drive it?  Or, can I get away with just using the ATMega IC on the PCB for the "brains", and then try and figure out which pins I need to solder to?

I would like to not have to purchase another Arduino to be permanently connected to the PCB, but do not know how to go about it.
